Please note that anything in [ ] must be changed appropriately as 
these are suggestions.  Please review your letter to make sure it 
makes sense before sending it out.  The list of legislators is under 
Public Health Committee members in this same area of 
'Pending Legislation'.

Dear [specify each legislator's name with the title of Sen. Or Rep.],

I am writing to you out of concern over Proposed Bill 5286.  Section 6
and 12 is of particular concern to me because [list your reason...ie. 
Contraindication, religious reasons].  I [or my child or family member]
would be adversely affected [share your reason] if he/she were forced 
to be vaccinated.

I would appreciate if you revise Bill 5286 to include a medical and
religious exemption and an option for humane quarantining. [You may
request that you would prefer to be quarantined in your home as the 
NY, MA, RI and several other states are proposing.]

Additionally, I want the definition of a public health emergency to 
be limited so that the governor could not declare a state of emergency 
for the flu every year.

I look forward to hearing your response as I will be following this 
legislation closely.
